This article discusses how to determine and agree on goals of care for a patient nearing the end of life. This topic brings together some of the previous material from the Education for Physicians on End-of-Life Care program. For example, advance directives and pain management are important issues to consider in arriving at goals of care. Frequently, patients and their families do not have advance directives and have not ever discussed end-of-life management. The case study presented below illustrates some of these points. CASE STUDY
