Stem cell technology is moving forward at a tremendous rate. Recent discoveries have surprised even the most expert researchers. While every piece of new data broadens the current knowledge and contributes to this moving forward, the new data also serve as paradigm shifters of fundamental knowledge of cell biology. While the question ‘What is a Stem Cell’ may now seem to basic to even discuss, there are still some discrepancies, however, between groups in terms of their functional roles. Teeth develop from the ectoderm of the first branchial arch and the ectomesenchyme of the neural crest. Deciduous teeth start to form between the sixth and eighth weeks, and permanent teeth begin to form in the twentieth weeks. Several studies have demonstrated that the pulp from both adult teeth and deciduous teeth contains dental pulp stem cells. Several factors have made them very attractive as a model system for many researchers; they are multipotent, ethically and non-controversially available in large numbers, immuno-compatible, developmentally primitive, easy to isolate and have high expansion potential in vitro. However, many controversies still exist in the field. There are several unanswered questions in the biology of dental pulp and odontoblasts. It will also discuss their therapeutic mechanisms that make them useful in dentistry and dental treatments. Over the past few decades, stem cell research has gained extensive scientific inquiry. This aspect is attributable to the significance of stem cells in tissue engineering. It is apparent that tissue regeneration has emerged as a reliable medical approach for the treatment of tissue disorders and injuries. Initially, embryonic stem cells were preferred as candidates for regenerative medicine because these cells can be induced to replicate in a pluripotent state. However, stem cell research involving embryonic stem cells has attracted immense controversy. It is also associated with legal and ethical issues, thus limiting the use of embryonic stem cells in regenerative medicine. Fortunately, the discovery of mesenchymal stem cells (MSCs), also referred to as adult stem cells, has restored promise for the development of stem cell therapies. Unlike embryonic stem cells, MSCs are free from legal and ethical concerns. MSCs are usually pluripotent progenitor cells that are generated in an array of tissues in both foetal and adult life. It is reported that these progenitor cells differentiate into cell types of the tissues that generate them, although studies indicate that they can differentiate cell types of other tissues. Currently, MSCs are used for regenerative therapies for a number of tissue disorders and injuries including bone regeneration. For instance, MSCs generated by the dental pulps and the oral cavity tissues have been found to possess the potential for dental tissue regeneration. These cells have also been found to useful in non-dental tissue repair.

Unspecialized stem cells can give rise to specialized cells including heart muscle cells, blood cells, or nerve cells. This process is called differentiation. Several types of stem cell have been discovered from germ cells, the embryo, fetus, and adult. Each of these has promised to revolutionize the future of regenerative medicine through the provision of cell-replacement therapies to treat a variety of debilitating diseases. Stem cell research opens-up the new field of 'cell-based therapies' and, as such, several safety measures have also to be evaluated. Regenerative medicine is based on stem cells. The dental pulp contains the progenitor/stem cells which have the self-renewal capability, multi-lineage differentiation capacity and clonogenic efficiency. Dental stem cells can generate many types of tissues including bone, nerve, cartilage, teeth, and fat. However, the biology of these mysterious cells have yet to be understood and a lot more basic research is needed before new therapies using stem-cell-differentiated derivatives can be applied. Recently stem cells banks are present, and even some of these banks do not only freeze cord stem cells but also dental stem cells of baby teeth. Hence this review attempts to study the history, current concepts, evolution of stem cells in dentistry, dental stem cell banking in India, characterization of the stem cells in dentin regeneration and stem cell therapy in dentistry.
