Aims: To determine the Incidence of Early Onset Neonatal Sepsis and the Incubation time for positive blood culture.Background: Neonatal sepsis is one of the leading causes of morbidity and mortality. Group B Streptococcus is one of the most common cause of early onset neonatal sepsis. Incidence has declined since the introduction of Intrapartum Antibiotic cover. However there is no single source of hospital Data in Australia.Methods: Descriptive study carried out for six years (01.01.2010- 31.12.2015) on all neonates born at Redland Hospital with risk of Infection. Blood culture results obtained from Auscare data were analysed using descriptive statistics via the use of Microsoft Excel Version 2013 to identify the frequency of clinically significant blood cultures and the Incubation time to positive growth of Bacteria. The Bac TAlert system was used to detect bacterial growth.Results: 11,977 babies were born between 01.01.2010-31.12.2015.Total of 640(5.34%) Blood cultures were collected.14/640(2.18%) had positive growth. Only 4/640 (0.62%) (95%CI 0.17-1.59%) had true positive growth responsible for clinical illness. All four grew Group B Streptococcus within 15 hours of Incubation (median 9.65 hours, inter quartile range 9.2-12.25hours).Others 10/640 (1.56%) were contaminant. Coagulase negative Staphylococcus was the most common contaminant. Conclusions: Incidence of Early Onset Neonatal Sepsis in our hospital is very low (0.62%).Mortality rate is zero. Median Incubation time to true positive growth of bacteria is 9.65 hours. We recommend stopping antibiotics at 24 hours in babies who remain clinically well and have negative cultures by 24 hours.

Moyosore 21 Neonatal Intensive Care, Rosie Hospital, Cambridge, UK2 Neonatal Intensive Care, Luton & Dunstable Hospital, Luton, UKBackgroundThe incidence of early onset neonatal sepsis is 0.9/1000 live births1. Introduction of National institute of Health and Clinical Excellent (NICE) guidelines in UK (2012) led to increase in babies being commenced on antibiotics, for longer durations and more undergoing LPu2019s. Meningitis is very uncommon in asymptomatic babies with only perinatal risk factors for sepsis2. Our unit uses first CRPu226520mg/l or second CRPu226550mg/l as guide for Lumbar punctures (LP).MethodsData was collected January-March 2017 for all neonates who were started on antibiotics having met our local criteria for screening.ResultsThere were 169 such episodes, 46% female and 54% male neonates. Gestation varied between 25+6-40+weeks and 76% were term.Major Indication for septic screen were for maternal temperature and antibiotics in 39% and 22% were for symptomatic neonates. Other risk factors contributed to the remaining.Only 1 positive blood culture (Streptoccocus Agalactiae) was seen in a symptomatic term neonate with normal CRPu2019s normal. 13.6% of neonates met our criteria for LP, however none of them were treated for suspected early onset neonatal meningitisu201d.64% had antibiotics for 2-3days, 30% for 4-7days and rest >7days None of the neonates treated for suspected early onset sepsis re-presented to paediatrics with suspected partially treated meningitis/sepsis in the first 10 days after being discharged from hospital ConclusionAlthough the criteria for performing an LP differs from that of NICE, we have shown that there were no cases of early onset neonatal meningitis in this study. We therefore propose that an arbitrary cut off value for CRP should not strongly influence the decision to perform an LP and instead we should rely on a combination of factors particularly the presence of symptoms. ConclusionsIn conclusion, it is safe to follow our NICU criteria for identifying at risk neonates and raising the CRPu2019s limits for otherwise well neonates has reduced need for LPu2019s without adversely impacting the outcomes for the neonates treated.References1)tVergnano S, Menson E, Kennea N, et al.
